Step 1: To learn how Connections to Care can support you with enriched aging, use the button below to schedule our 15 minute complimentary phone conversation. 

Step 2: Following the 15 minute complimentary phone conversation, one of our Care Managers will schedule an agreed upon time to complete an initial assessment.

Step 3: The initial assessment is usually completed where the aging adult lives and can include family members/caregivers assisting with care. Initial assessment fees apply. 

Care Management:


Once the initial assessment has been completed, our Care Manager will formulate an individualized care map for the client and/or caregivers. The individualized care map will include identified needed services from initial assessment to include organization names, contact information to include point of contact, services offered and pricing (if applicable). The Care Map will be reviewed with the client and caregiver either in person or via video conference.

Written Individualized Care Map fees apply. 

Our Care Managers can execute any or all of the Individualized Care Map services as directed by the client and/or family. The Care Manager will be there from the onset of services and continue with monthly check-in to validate services continue to meet client and/or family needs.

Placement Services:

Aging at home is preferred in many situations over moving to a care community. However, there are times when an Assisted Living or Independent Living community is appropriate to enhance aging well. The Care Managers at Connections to Care are experts in facility operations, state and federal regulations and guiding families through the myriad of departments. Our astute knowledge of care communities takes the stress out of locating the right supportive environment for your loved one. Through our professional collaborations with community directors and marketing staff, Connections to Care can offer a variety of senior living community options at all price points. We assist aging seniors and their families with transition from home to a community setting with grace, compassion and expertise. Initial assessment fees may apply. Connections to Care is reimbursed by senior living communities for placement services.

We advocate for our clients living in care communities. Adult children who live out of town can partner with Connections to Care to visit your loved one, attend Care Coordination meetings on the families behalf, and help with transitions from hospital back to community are a few of the co-managment services available.

SC Medicaid Community Based Waiver Services:

All participants must be eligible for South Carolina Medicaid Home and Community Based Waivers. Eligible participants have chosen to receive care at home instead of a facility setting. 

Case Managers help clients and families set up services with Medicaid approved providers. Assessed needs can be, but are not limited to, the following: 

  • Personal Care Aides (PCI/PCII)

  • Pest Control

  • Environmental Modifications and Ramps

  • Personal Emergency Response System

  • Adult Day Health Care & Transportation 

  • Home Delivered Meals

  • Nutritional Supplements

  • Incontinence Supplies

Connections to Care is a provider of South Carolina Medicaid waiver services for Community Long Term Care. We provide Case Management for the following waivers: Community Choices, HIV and Ventilators.
